Regulations for the provision of electronic services

§ 1. General provisions

These Regulations for the provision of electronic services specify the scope and conditions for arranging and booking appointments via the website:

Definitions:

Price list – the price list of Services, made available to the Patient on the Service Provider’s website,

Patient – a natural person who uses the services provided electronically by the Service Provider,

Regulations – these regulations for the provision of electronic services, made available free of charge on the website www.osrodekneurorehabilitacji.pl in a form that allows its downloading, recording and printing, so as to enable free familiarization with the provisions of the Regulations before accepting them,

Website – the website of the Service Provider, used to arrange and book appointments, available at www.osrodekneurorehabilitacji.pl

Service – a service provided by the Service Provider, consisting in providing the Patient with the possibility of arranging and booking an appointment by electronic means,

To be able to use the Service, you must read the Regulations carefully and accept its terms. Failure to accept the terms of the Regulations makes it impossible to use the Service.

Other services, i.e. a visit, take place in person at the office, at the time and place indicated when booking the date. The conditions for the provision of other services are not covered by the Regulations.

§ 2. How to use the services provided and fees

Booking the date of the visit is done via the registration form and the schedule posted on the Website in the “Make an appointment” tab.

A patient booking a visit is obliged to:

complete the form with the required data: name and surname (patient), telephone number and e-mail address (parent/ward);

pay a fee of 100% of the visit price indicated when choosing the date of the visit.

Providing data by the Patient is tantamount to submitting his statement about the truthfulness of all data provided to the Service Recipient and having full legal capacity, or in the case of limitation, having the necessary consent of the legal guardian.

After completing the registration form, a link to the payment page will be sent to the e-mail address provided by the Patient in the registration form.

The condition for booking a visit is to pay a fee of 100% of the price of the visit resulting from the Price List. The payment is considered to have been made only after it has been credited to the Service Provider’s bank account or the payment has been confirmed by PayU.

If the fee is not paid up to 24 hours before the selected date of the visit or immediately in the case of booking a date in less than 24 hours to the selected date of the visit, the date is considered unbooked and available to other potential patients.

The Patient will immediately receive information about the booking of the payment and confirmation of the reservation or cancellation due to the lack of prepayment to the e-mail address provided in the registration form.

§ 3. Complaints

The Patient has the right to submit complaints related to the Service via e-mail to the e-mail address of the Service Provider indicated in these Regulations.

The deadline for considering the complaint is a maximum of 14 working days from the date of receipt of the complaint.

§ 4. Cancellation or rescheduling of the visit

Cancellation of a visit within the booked date is possible no later than 24 hours before this date. If you do not appear at the appointment and there is no cancellation within the specified time, the fee paid is not refundable.

Cancellation of the booked visit date is possible by sending an e-mail to the e-mail address of the Service Provider provided in the Regulations. The Service Provider does not provide any other possibility to cancel the reservation.

Failure to appear at the scheduled appointment or failure to appeal within the indicated period results in forfeiture of the fee. The fee is non-refundable regardless of the reason.

If the Patient is late for the booked date of the visit, if it takes place, it results in its shortening by the time of delay, while the fee remains unchanged. In the event of a delay exceeding half of the planned duration of the visit, the Service Provider is entitled to cancel the booking of the visit, which results in the forfeiture of the fee. The fee is non-refundable regardless of the reason for the delay.

The Service Provider reserves the right to cancel the booked visit at least one hour before the date. In the event of cancellation, the Service Provider will propose a new appointment date to the Patient. In case of not accepting the new date of the visit or not proposing it, the Servicesfrom
